Device Identification

  • Checkbox
    Record device make, model, serial number, and size
  • Checkbox
    Verify device type (DCVA, RPZA, PVB, DCDA)
  • Checkbox
    Record device location and service description
  • Checkbox
    Confirm device is the correct type for the hazard level
  • Checkbox
    Check DEP registration number matches installed device

Visual Inspection

    Inspect device for physical damage or corrosion

    Check for leaks at all connections and test cocks

    Verify shutoff valves upstream and downstream are operational

    Inspect relief valve discharge (RPZA) for continuous flow indicating failure

    Check that device is accessible for testing and maintenance

    Verify test cocks are present and functional

Differential Pressure Testing

    Connect calibrated differential pressure test kit

    Test first check valve for proper closure and record differential pressure

    Test second check valve for proper closure and record differential pressure

    Test relief valve opening point (RPZA) and record differential pressure

    Verify all readings meet minimum performance standards

    Record upstream supply pressure

Pass/Fail Determination

    First check valve holds minimum 1.0 PSI differential (DCVA) or 5.0 PSI (RPZA)

    Second check valve holds minimum 1.0 PSI differential

    Relief valve opens at minimum 2.0 PSI above second check closing point (RPZA)
  • Checkbox
    Air inlet opens at 1.0 PSI above atmospheric (PVB)
  • Checkbox
    Check valve holds minimum 1.0 PSI above atmospheric (PVB)
  • Checkbox
    Mark device as PASS or FAIL based on all test results

Reporting and Compliance

    Complete NYC DEP Backflow Test Report form
  • Checkbox
    Record all test gauge readings and serial numbers
  • Checkbox
    Submit test report to DEP within 30 days of test date
  • Checkbox
    Provide copy of test report to building owner or manager

    Schedule repair and retest if device failed
  • Checkbox
    Update device testing records and next due date
